Can someone inform me about the bankruptcy adversary proceedings form?

I’m not sure what the “origin of proceedings” should be. Our bankruptcy has been discharged, but we are seeking discharge of our student loans by reason of financial hardship.
Would this be considered: original prodeeding, removed proceeding, or reinstated or reopened.
Please only answer if you know what you’re talking about!
The bankruptcy discharge in a chapter 7 case closes the case, I believe.
We filed pro se, and plan to do the same with the adversary proceeding.
I really don’t need a lawyer for this, just a little information.

Has you bankruptcy case closed yet? You say you have received your discharge, but you didn’t say whether the case has closed. If the case has not closed, then the adversary proceeding would be a separate proceeding in the original case. If the case has closed, then you would need to reopen the case so that you could proceed with the adversary proceeding. You really need to see an attorney about this.
